Ministers Family Benefit P. O. Box 12609 The Pentecostal Holiness Ministers Family Benefit (MFB) is a cooperative benevolence program for Pentecostal Holiness ministers, missionaries, and their spouses, who are eligible to join if they are 55 years of age or under. An application form may be requested from the local conference office or from the general Ministers Family Benefit Office in Oklahoma City at the address listed above. To join, an initial fee of $5.00 is paid by each applicant and submitted with their application for membership to their conference. After joining, each member then pays $3.00 upon the death of a MFB member. This will be reported quarterly by the conference treasurer and is paid by the member directly to the conference. Upon the death of a member, the family may notify Stewardship Ministries and send a copy of death certificate. A check for $5,000 will then be paid to the beneficiary designated on the application form. This program is for the purpose of providing financial assistance to the beneficiaries of members and expressing brotherly love and concern for the bereaved family of a fellow minister.
